About The Role
You're the first “welcome” our patients feel. At Bloom Healthcare, you won't just process referrals — you'll own the entire journey from the moment a patient enters our world to the moment they're sitting across from their care team for the first time. You'll build their chart, dig into their medications, verify their insurance, and map out exactly what their care should look like. Then you'll go a step further: partnering directly with the patient's IDG team, working through the details together to make sure nothing is missed and that patient walks into their first visit fully set up for success.
You'll be on the phone with patients and the people who love them, answering questions, easing nerves, and making a healthcare system that can feel overwhelming actually feel human. You'll chase down referral sources, keep new patients moving through the pipeline, and be the person who notices when something needs attention before anyone else does. This isn't a role where you complete tasks and hand them off — it's a role where you drive the process, think critically, and take real ownership of getting vulnerable patients the care they need.
Responsibilities
- Own the patient intake journey from initial referral to first visit
- Build patient charts, review medications, and verify insurance details
- Collaborate with the patient’s interdisciplinary group (IDG) team to ensure comprehensive care planning
- Communicate directly with patients and families to answer questions and ease concerns
- Manage referral sources and ensure timely progression through the intake pipeline
- Identify and address potential issues proactively to ensure smooth patient onboarding
Requirements
- Bachelor’s degree
- 2+ years of customer service experience in an office setting
- Healthcare experience is a plus, but not required

Pay
The pay range for this role in the state of Colorado typically falls between $22–$26 hourly. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
