Responsibilities

  • Conduct thorough physical examinations and assessments for patients of all ages, including children ages 3 years and up.
  • Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma.
  • Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
  • Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs.
  • Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans.
  • Prescribe medications, recommend treatments, and provide written home-going instructions.
  • Refer patients to specialists or other healthcare resources as needed.
  • Educate patients and their families on health maintenance and preventive care.
  • Promote patient engagement, health literacy, and shared decision-making.
  • Participate in quality improvement initiatives and value-based care programs.
  • Emphasize patient-centered care, continuity of care, and building strong patient-provider relationships.
  • Ability to provide quality care to meet patient volume goals, expectation of +18 daily on average.
  • Preferred experience working as a Telehealth Clinician.
  • Lead clinic staff (e.g., X-Ray Technologists, Nurses, and Front Office Specialists) in delivering excellent patient care.
  • Provide guidance to ensure quality professional services and excellent patient satisfaction.
  • Discuss and review patients’ medical history, symptoms, allergies, and current medications.
  • Actively engage with clinical leadership, elevating concerns to management to ensure strong patient care and adherence to company values.
  • Ensure accurate completion of patient charts in a timely manner and forward charts as appropriate on a daily basis.
  • Work efficiently in a fast-paced, autonomous environment.
  • Dedication to evidence-based medicine and staying current with medical advancements.
  • Implement clinical protocols as outlined by the Chief Medical Officer and Supervising Physicians.
  • Analyze and interpret patients' histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses.
  • Deliver excellent patient care through in-person diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliably accurate information.
  • Ensure activities are conducted in compliance with Federal and State regulations, guidelines, and requirements, including HIPAA.
  • Complete all mandatory compliance training to meet and exceed our continued quality of care.

Requirements

  • Master’s degree in Nursing (MSN) and/or Master’s degree in Physician Assistant Studies (MMS).
  • 1 year of dedicated experience in direct primary care settings as a provider.
  • Proficient in providing comprehensive primary care, including chronic care management, annual wellness visits for adults and pediatrics, and preventive health measures.
  • Understanding of HEDIS quality metrics related to preventative screenings, vaccinations, and chronic conditions.
  • Required to be proficient in performing pediatric exams and see children aged 3 years and up.
  • Current, active license and DEA in good standing, verifiable with the proper regulatory agency.
  • Must be able to treat all ages and meet any credentialing requirements needed.
  • DOT certification required to be obtained within 120 days of employment and maintained during employment.

Qualifications

  • Family Nurse Practitioner License, Physician Assistant License, APRN, or Drug Enforcement Administration License.
