Responsibilities

  • Deliver high quality health care within Fast Pace Health’s scope of services while achieving optimum patient satisfaction.
  • Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma.
  • Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
  • Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs.
  • Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans.
  • Prescribe medications, recommend treatments, and provide written home-going instructions.
  • Refer patients to specialists or other healthcare resources as needed.
  • Educate patients and their families on health maintenance and preventive care.
  • Promote patient engagement, health literacy, and shared decision-making.
  • Participate in quality improvement initiatives and value-based care programs.
  • Emphasize patient-centered care, continuity of care, and building strong patient-provider relationships.

Essential Conduct

  • Provide quality care meeting patient volume goals, expected to see +18 patients daily on average.
  • Lead clinic staff, in behaviors, actions, and attitude (e.g., X-Ray Technologists, Nurses, and Front Office Specialists), to deliver excellent patient care.
  • Engage actively with clinical leadership, elevating to management where appropriate, to ensure strong patient care and resolution of concerns.
  • Achieve accurate completion of patient charts in a timely manner and forward charts as appropriate on a daily basis.
  • Work efficiently in a fast-paced autonomous environment.
  • Dedicate to evidence-based medicine and stay current with medical advancements.
  • Implement clinical protocols as outlined by CMO and Supervising Physicians.
  • Analyze and interpret patients' histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses.
  • Deliver excellent patient care through in-person diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliable accuracy.
  • Ensure activities of the Primary Care Provider are conducted in a manner consistent with overall department expectations and are in compliance with Federal and State regulations, guidelines, and requirements.

  • Communicate regularly within team in all manners necessary to support excellent patient care.
  • Build and maintain confidence and credibility with all employees.
  • Maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in a positive work relationship with customers, co-workers, and managers.
  • Perform the physical, use of senses, cognitive, and environmental functions of the position as specified on the physical demands.
  • Assist with urgent care activities as primary care schedule allows.

Experience Requirements and Preferences

  • Nurse Practitioner (NP) / Physician Assistant (PA) with 1 year of dedicated experience in direct primary care settings as a provider.
  • Proficient in providing comprehensive primary care, including chronic care management, annual wellness visits for adults and pediatrics, and preventive health measures.
  • Understanding of HEDIS quality metrics related to preventative screenings, vaccinations, and chronic conditions.
  • Current License or Certification: Family Nurse Practitioner License, Physicians Assistant License, APRN, Drug Enforcement Administration License.
