Core Responsibilities
- Conduct in-person comprehensive medical assessments
- Partner with the interdisciplinary team to design and implement individualized care plans
- Manage discharge planning for participants transitioning from acute or long-term care settings
- Oversee participant care within nursing facilities, providing regular clinical visits
- Facilitate effective care coordination with community-based physicians and specialists
- Analyze utilization data and hospitalization outcomes to improve clinical policy and best practices
- Participate in a rotating schedule for night and weekend on-call coverage
Requirements
- Candidate Qualifications:
- Degree from an accredited School of Nursing (NP) or Physician Assistant (PA) program
- Current California State License as a Nurse Practitioner (with prescriptive authority) or Physician Assistant
- Minimum of two years of clinical experience specializing in the geriatric population
- Strong proficiency in chronic care management and geriatric best practices
