Primary Care Provider

About the role

The Primary Care Physician (PCP) provides comprehensive care to patients, focusing on acute and chronic illnesses, especially in older adults. They are accountable for patient outcomes, practicing evidence-based medicine, and acting as health coaches to promote behavior change and better health.

Responsibilities

• Independently assess, diagnose, treat, and manage patient health needs, coordinating with specialists and hospital teams as necessary.
• Lead care teams, including medical assistants and care coordinators, and engage with patients in various settings such as in-office, hospital, or home care.
• Contribute to center management, support cultural initiatives, and participate in risk and quality programs.
• Ensure compliance with standards, policies, and regulations while striving for high-quality patient care and improved health outcomes.
• Engage in recruiting and covering for colleagues as needed.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar; board eligibility required, board certification preferred.
• Active medical license and DEA number; BLS certification required within 90 days.
• Strong leadership, communication, and relationship-building skills, with proficiency in evidence-based practices and commitment to physician wellness.
• Ability to work independently and collaboratively, with willingness to travel locally and regionally.

$214,700 to $306,714
