Primary Care Physician | Value-Based Care | $230K–$250K

Heartline Staffing is recruiting a Primary Care Physician to provide value-based healthcare to Medicare patient populations, supported by large care teams to focus on quality of care rather than volume of services.

About the role

• Value-Based Care model focused on outcomes, reducing hospital admissions, preventive medicine, and patient experience
• Medicare patient population
• 13-16 patients per day
• 20-minute regular visits and 40-minute full diagnostic and new patient visits
• Care team includes a Scribe, Medical Assistant, and Social Worker
• On-site labs with a phlebotomist
• Access to RubiconMD for specialist consultations
• Average of 3-5 providers per clinic, including NPs and Primary Care Physicians
• EMR systems: Canopy and Greenway

Responsibilities

• Provide value-based healthcare to Medicare patient populations
• Focus on quality outcomes and preventive medicine
• Work within a care model designed to reduce hospital admissions
• Collaborate with a multidisciplinary care team

Requirements

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate
• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Active, non-probationary, unrestricted state license
• 2-5 years of experience
• No residents

Preferred Qualifications

• Fellowship training in Geriatrics

Pay

• $230,000–$250,000 base salary
• Bonus based on quality metrics
• Sign-on flexibility and relocation package considered on a case-by-case basis

Schedule

• Monday–Friday, 8:00 AM–5:00 PM
