Primary Care Physician | Adults 65+ | $250K–$260K

Heartline Staffing is recruiting a Primary Care Physician to provide preventive primary care for adults aged 65 and older within a value-based care model. The patient population is primarily Medicare Advantage, with a focus on chronic disease management and preventive care.

About the role

• Provide preventive primary care and chronic disease management for adults aged 65+
• Collaborate with an integrated care team including NPs, PAs, medical assistants, social workers, lab and pharmacy support, and referral coordination
• Approximately 15 patients per day
• No RVU pressure
• Value-Based Care experience preferred but not required; training provided

Responsibilities

• Provide preventive primary care for adults 65+
• Focus on chronic disease management and preventive care
• Practice within a Value-Based Care model
• Work with an integrated care team to coordinate patient health

Requirements

• Board Certified or Board Eligible in Internal Medicine or Family Medicine (required)
• Geriatric experience (preferred)
• Value-Based Care experience (preferred; training provided if needed)
• HEDIS experience (a plus)

Pay

• $250,000–$260,000 base salary
• Competitive sign-on bonus
• Annual performance bonus: 20% of base salary
