Bilingual | Primary Care Physician | Value-Based Care | $230K–$250K

Heartline Staffing is recruiting a Primary Care Physician to provide value-based healthcare to Medicare patient populations, focusing on quality outcomes, preventive medicine, and reducing hospital admissions.

About the Role

• Value-Based Care model serving Medicare patient populations
• 13–16 patients per day with 20-minute regular visits and 40-minute full diagnostic/new patient visits
• Collaborative care team including a Scribe, Medical Assistant, and Social Worker
• On-site labs with a phlebotomist and access to RubiconMD for specialist consultations
• EMR systems: Canopy and Greenway
• Clinics staffed by an average of 3–5 providers, including NPs and Primary Care Physicians

Responsibilities

• Provide value-based healthcare with a focus on quality outcomes and preventive medicine
• Work within a care model designed to reduce hospital admissions
• Collaborate with a multidisciplinary care team to manage patient health
• Utilize on-site lab services and specialist access via RubiconMD

Requirements

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate
• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Active, non-probationary, unrestricted state medical license
• Spanish-speaking proficiency
• 2–5 years of experience

Preferred: Fellowship training in Geriatrics.

Compensation

• $230,000–$250,000 base salary
• Bonus based on quality metrics
• Sign-on flexibility and relocation package considered on a case-by-case basis
