About the role

Primary Care Physician responsible for providing equitable, value-based healthcare to Medicare patients in a neighborhood primary care setting. Focuses on delivering high-quality, preventive care while reducing hospital admissions, with performance driven by outcomes and bonuses. Collaborates with care teams and leadership to meet quality goals and manages patient records using electronic systems.

Responsibilities

• Assess, diagnose, and treat patients
• Prescribe medications
• Maintain electronic medical records
• Collaborate with leadership and care teams
• Manage center operations as needed

Requirements

• MD or DO graduate with internal medicine or family medicine certification (or eligible)
• Unrestricted state license and DEA registration
• US work authorization

Preferred Skills

• Geriatrics fellowship or experience with older adult populations
• Experience in value-based healthcare and outpatient primary care
• Bilingual abilities where applicable

Schedule

Full-time role averaging 40 hours per week.
