About the role
Primary Care Physician responsible for delivering equitable, value-based healthcare to Medicare patients in a neighborhood primary care setting.
Responsibilities
- Assess, diagnose, and oversee primary care for patients
- Prescribe medications and treatments
- Maintain electronic medical records using specified platforms
- Collaborate with regional leadership to meet quality goals
- Manage and direct the care team in coordination with Practice Managers
- Perform other related duties as needed
Qualifications & Skills
- Medical degree (MD or DO) with board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state license and DEA license
- US work authorization
- Preferred: Geriatrics fellowship, experience with older adults, value-based care, outpatient primary care, bilingual abilities
- Motivated to provide excellent care to older populations, including new graduates
Additional Details
Full-time role with approximately 40 hours per week
