Responsibilities

• Deliver high-quality, preventative primary care to adult Medicare patients across multiple clinics.
• Manage chronic conditions and address acute health needs within a team-based, data-informed model.
• Collaborate with nurses, care managers, and social workers to close care gaps and improve patient outcomes.
• Travel regionally to provide consistent, comprehensive care.

Requirements

• Medical degree with appropriate licensing.
• Strong clinical skills in primary care, chronic disease management, and acute care.
• Excellent teamwork and communication abilities.
• Ability to travel regionally and work in a patient-centered environment.
