Primary Care Physician - Spanish Speaking Preferred

Responsibilities
- Demonstrate accountability for clinical outcomes, cost-effectiveness, and strong clinical care for a panel of up to 450 patients.
- Act as a health coach by helping patients set and achieve short- and long-term health goals, building trusting relationships, and frequently following up on progress.
- Simplify and prioritize care plans to make behavior change actionable for patients and care teams.
- Work toward key performance metrics, including patient admissions per thousand, appointment slot utilization (18-21 slots/day), CGCAHPS (patient experience), clinical gap closures, and medical cost measures.
- Champion a positive culture of love, accountability, and passion within the medical center.
- Function both independently and collaboratively with healthcare professionals, ensuring compliance with guidelines and participating in risk and quality management programs.
- Assess acute and non-acute clinical problems, perform physical assessments, document patient histories, and develop patient management plans.
- Write admission, transfer, and discharge orders; order and interpret laboratory and diagnostic studies; prescribe medications and treatments; and refer patients to specialists as needed.
- Document in-depth progress notes and summaries, and perform invasive procedures upon documented competency.
- Participate in patient care rounds, conferences, and multidisciplinary team collaborations to ensure successful patient care strategies.
- Recognize and respond to medical emergencies, initiating life-saving procedures when necessary.
- Collaborate with multidisciplinary teams to improve healthcare delivery systems and implement new practice patterns.
- Facilitate patient/family conferences to review treatment goals, optimize resource utilization, and plan post-care transitions.
- Enhance collaborative relationships to support informed patient/family decisions regarding goals of care, palliative care, and hospice.
- Prescribe medications in accordance with state practice regulations.
- Complete additional duties as assigned, including transitioning patients to lower levels of care and arranging SNF discharges.
Requirements
- ARNP or similar advanced degree in Nursing required.
- Current Nurse Practitioner Certification in the State of practice required.
- Board certification by AANP or ANCC preferred but may be required for certain states.
- Current DEA number for schedule II-V controlled substances may be required based on State of practice.
- A minimum of 1 year of clinical experience preferred; strong new graduates may be considered.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment.

Pay
The posted pay range for this position is $221,141 - $315,915 annually. Final compensation will depend on factors such as experience, education, geographic location, and other relevant considerations. This position may also be eligible for bonuses or commissions.
