Primary Care Physician / Medical Doctor

Responsibilities

• Collect, record, and maintain patient information.
• Prescribe or administer treatment, medical therapy, medication, vaccination and other specialized psychiatric medical care to treat or prevent illness, disease or injury.
• Order, perform and interpret tests and analyze records, reports, and examination information to diagnose a patient's condition.
• Monitor patient's conditions and progress and reevaluate treatments as necessary.
• Explain procedures and discuss test results or prescribed treatments with patients.
• Refer patients to medical specialists or other practitioners as necessary.

Requirements

• D., D.O. required.
• Current Board Certification in Internal Medicine, Family Practice, or Pediatrics, or eligible to be Board certified.
• Prescriptive Authority in Indiana (DEA License).
• CPI (Crisis Prevention Intervention) - certification must be obtained after hire and prior to providing direct patient care.
• BLS (Basic Life Support) and First Aid - certification must be obtained after hire and prior to providing direct patient care.
• Valid driver’s license and active auto insurance coverage.
• Satisfactory results from criminal and driving record background checks.

Qualifications

Education: D., D.O. required.

Certificate or Professional License: Current Board Certification in Internal Medicine, Family Practice, or Pediatrics, or eligible to be Board certified. Prescriptive Authority in Indiana (DEA License).
