Responsibilities
- Mentor and lead a care team
- Provide high-quality primary care, including addressing medical and social needs
- Collaborate with a supportive care team
- Be available to patients
- Manage a small panel size to build strong relationships with patients
- Work in outpatient primary care settings
- Deliver care to those with comorbid conditions, the elderly, and/or medically underserved populations
- Drive a passion for work and be energized by being part of a fast-growing organization
Requirements
- M.D. or D.O. graduates
- Board certified/eligible in Internal Medicine or Family Medicine (required)
- Fellowship training in Geriatrics welcome, but not required
- Active, non-probationary state medical license
