Job Summary
Primary Care Physicians must provide direct patient care to our Veteran population located at the Brainerd CBOC in Brainerd, MN. Primary Care Physicians are required to deliver high-quality, compassionate, and consistent care to individuals.
Duties
- Performing annual physicals, screenings, and vaccinations to maintain good health and prevent disease.
- Treating common illnesses and minor injuries, such as colds, flu, rashes, and sprains.
- Helping patients manage long-term health conditions like hypertension, high cholesterol, and diabetes through regular check-ins, medication management, and ordering tests.
- Providing counseling and patient education on healthy lifestyle choices, nutrition, and weight management.
- Diagnosis, evaluation, treatment, care management, and consultation across all settings.
- Providing consultation with and instruction to referring physicians regarding appropriate indications for procedures.
- Determining the appropriate course of treatment and communicating in person or by phone with referring clinicians.
- Initiating appropriate social work referrals for all identified homeless veterans and for patients who do not have primary care providers.
- Providing discharge education and follow-up instructions that are coordinated with the next care setting.
- Communicating test results.
- Completing appropriate QM/PI documentation pertaining to all procedures, complications, and outcome of examinations.
- Patient documentation completed using the current Electronic Health Record (EHR).
- Selecting appropriate Evaluation and Management (E&M) procedure codes in CPRS.
- Following all established patient safety and infection control standards of care.
- Making every effort to prevent medication errors, falls, and patient injury caused by acts of commission or omission in the delivery of care.
- Refer all patient/customer service issues to the Supervisory Physician, Nurse Manager, or designee.
Qualification(s)
- A current CPR/BLS certificate by the American Heart Association is required.
- Active, current licensure to practice medicine in any state, territory, or Commonwealth of the US.
- Board Certification/Eligible.
- DEA.
- Completion of an accredited residency program in Internal Medicine or Family Practice.
