Responsibilities
- Primary care physicians transforming the health and lives of seniors on Medicare Advantage through Value Based Care
- High-touch, relationship-centered model that supports patient and provider success
- Small patient panel of up to 450 patients
- No nights or weekends, 100% outpatient practice

Requirements
- Board certified or board eligible in FM, IM, or Geriatric Medicine
- Open to residents and fellows
- Open to visa candidates

Pay
The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
