Primary Care Outpatient APRN I Complex Care Team

About the Role

Join a specialized team providing individualized care for patients with complex medical needs, including sickle cell disease. This role focuses on coordinating care through education and medication management to help patients gain long-term control over their health. Knowledge of sickle cell disease is not required.

This opportunity offers a low patient volume (approximately 8 patients per day) with no call, nights, or weekends. Collaborate with a multidisciplinary team to make a lasting impact on patients' lives.

Responsibilities

• Clinical Care
  • Provide assessment, triage, and therapeutic management of patient and family needs as part of an interdisciplinary team.
  • Obtain medical histories, perform physical examinations, order and review diagnostic tests, and formulate patient treatment plans in collaboration with supervising physicians.
  • Perform procedures, prescribe treatments and medications in accordance with institutional and state guidelines.
  • May function as first or second assistant in operative cases, if within scope and job prerequisites.
  • Respond to emergency situations within YNHHS parameters.
• Patient Education & Psychosocial Support
  • Educate patients and families on care plans, treatment side effects, and available resources.
  • Provide psychosocial support to patients and families in a caring, ethical, and professional manner.
• Information Management
  • Ensure accurate and compliant documentation in accordance with hospital and regulatory requirements.
  • Protect patient information from unauthorized access or disclosure.
• Quality Management
  • Demonstrate high standards of ethical behavior, professionalism, and compassion in patient care.
  • Ensure compliance with healthcare regulatory requirements and standards.
  • Report safety events and near misses to support high-reliability organizational behaviors.
• Professional Development
  • Engage in projects to maintain and advance professional competency.
  • Serve as a clinical resource, preceptor, or mentor for students and new staff.
  • Collaborate on research, quality assurance, and performance improvement projects.
  • Maintain all employee requirements as determined by hospital policy.

Qualifications

• Education: Graduate from an accredited Nurse Practitioner or Physician Assistant Program.
• Experience: Clinical experience as an NP/PA within the specialty is preferred but not required.
• Licensure/Certification:
  • Active certification by the National Commission of Certification of Physician Assistants (NCCPA) or Connecticut State Nurse Practitioner License with board specialty certification.
  • Connecticut State Physician Assistant License or Connecticut State Registered Nurse License.
  • ACLS/BLS certification (when indicated for position profile).
  • DEA License (when indicated for pediatric positions).
  • PALS (for neonatal care) or NRP Certification, as applicable.
  • Additional licensure, certifications, or continuing education credits may be required based on specialty.
• Appointment to the Affiliated Medical Staff of a YNHHS Hospital and credentialing through the Medical Staff process is required.
• Evidence of current competence to practice must be provided during the Medical Staff appointment process.

Schedule

Full-time position with scheduled hours from 8:30 AM to 4:30 PM, Monday through Friday.
