About the role
The Veterans Healthcare System of the Ozarks seeks a Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) for the Jay-Oklahoma location.
Responsibilities
- Delivers fundamental - knowledge-based care to assigned clients while developing technical competencies.
- Utilizes registries and ensures appropriate evaluation and access is provided to patient panels.
- Manages assigned panel(s).
- Provides health promotion, health education, and coaching on wellness, disease prevention, and chronic care management.
- Administers medications and procedures per established policies and guidelines.
- Influences care outcomes by collaborating with members of the interdisciplinary team.
- Affords assistance in directing the provision of nursing education, orientation, competencies, and provides quality improvement and outcomes utilization.
- Improves Veteran/patient-driven care by resolving experience issues such as coordination of care, timeliness, or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting.
Requirements
- Basic Requirements: English Language Proficiency, Graduate of an approved nursing program, Current full active unrestricted registration as a graduate professional nurse, Possession of a current full active unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
- Nurse I - Level I: Associate Degree (ADN) or Diploma in Nursing, Nurse I - Level II: ADN or Diploma in Nursing AND 1 year of specialized nursing experience, Nurse I - Level III: ADN or Diploma in Nursing AND 2 years of professional nursing experience, Nurse II: BSN with 2 years of professional nursing experience, Nurse III: MSN and 2 years of specialized nursing experience, Doctoral degree in Nursing with 1 year of specialized nursing experience.

Skills
- Ability to lift 45 pounds and carry 45 pounds, ability to walk 7 hours, stand 7 hours, reach above shoulder, use fingers, both hands required, kneel 0.5 hours, bend 7 hours, both legs required, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13"" to 16"" to Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other, ability to distinguish basic colors, ability to distinguish shades of colors, hearing without aid, mental and emotional stability, work with detergents and antiseptic solutions.
