Primary Care Office Nurse (LPN)

About the role

Supports the overall health and well-being of residents by delivering high-quality, person-directed care and nursing services within the Primary Care Office.

Contributes to interdisciplinary care, supports proactive health monitoring, and facilitates access to well-being resources.

Fosters a holistic, resident-centered approach that promotes independence, dignity, and thriving in place.

Responsibilities

• Deliver high-quality, person-directed care and nursing services within the Primary Care Office.
• Support proactive health monitoring and access to well-being resources.
• Collaborate with the Nurse Practitioner and the broader wellness team to advance the Acts Signature Well-Being Model.
• Contribute to interdisciplinary care and support resident independence, dignity, and thriving in place.

Requirements

• Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing.
• Minimum of two years of clinical nursing experience, preferably in a senior living, primary care, or wellness-focused environment.
• Prior experience in office management, care coordination, or leadership roles is strongly preferred.
• Current certification in CPR, AED, and first aid (or obtain within three months of employment).

Pay

$24.72 - $31.97 / hour. Starting rate will vary based on skills and experience.
