About the role

Supports the overall health and well-being of residents by delivering high-quality, person-directed care and nursing services within the Primary Care Office. Contributes to interdisciplinary care, supports proactive health monitoring, and facilitates access to well-being resources.

Responsibilities

  • Deliver high-quality, person-directed care and nursing services within the Primary Care Office.
  • Contribute to interdisciplinary care, supporting proactive health monitoring.
  • Facilitate access to well-being resources.
  • Collaborate closely with the Nurse Practitioner and the broader wellness team to foster a holistic, resident-centered approach.

Requirements

  • Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing.
  • Minimum of two years of clinical nursing experience, preferably in a senior living, primary care, or wellness-focused environment.
  • Prior experience in office management, care coordination, or leadership roles is strongly preferred.
  • Current certification in CPR, AED, and first aid (or obtain within three months of employment).
