Primary Care Nurse Practitioner

Role: Nurse Practitioner in an outpatient clinic setting. Responsibilities Assess, diagnose, and manage acute and chronic conditions.Order and interpret diagnostic tests.Prescribe medications and develop evidence-based care plans.Collaborate with physicians, nurses, and interdisciplinary teams to ensure patient-centered, culturally competent care.Provide patient education and coordinate care.Document clinical information in the EHR and participate in quality and safety initiatives.Contribute to a supportive, learning-focused clinical environment aligned with community health goals.
