Primary Care Nurse or LPN

Seeking a Primary Care Nurse to collaborate with providers and the care team, delivering patient-centered, safe and effective care. Responsibilities include managing triage calls/messages, supporting quality outcomes, and working within our mission and standards. The role offers a Monday to Friday schedule with no nights, weekends, or holidays. After an initial training period, the position features a 50/50 hybrid work model, requiring on-site presence for training and monthly meetings. Qualifications Licensed Practical Nurse (LPN) with a diploma and active license, 2-3 years preferred.Registered Nurse (RN) with an associate, bachelor’s degree, or equivalent experience, with an active license and 2-3 years preferred. Preferred Skills Proficiency with Epic EMR, strong triage and computer skills, excellent time management, and adaptability to change.
