Primary Care Nurse or LPN

Role: Primary Care Nurse, working under the primary care operations team to deliver safe, effective, and patient-centered care in collaboration with providers and the clinical team. Key Responsibilities: Manage triage calls/messages, partner with patients and providers to drive quality outcomes, and perform duties aligned with mission and standards. After an initial training period, transition to a hybrid work model with on-site and remote work, attending monthly clinic meetings. Qualifications Licensed Practical Nurse (LPN): Diploma in Practical Nursing, active LPN license, 2-3 years of preferred experience.Registered Nurse (RN): Associate or bachelor’s degree in nursing, active RN license, 2-3 years of preferred experience. Preferred Skills: Proficiency with Epic EMR, excellent triage and computer skills, strong time management, adaptability, and ability to work in a changing environment.
