Primary Care Nurse or LPN

We are seeking a Primary Care Nurse to collaborate with Primary Care Providers in delivering patient-centered, efficient, and equitable care. Under the direction of the primary care operations team, the nurse will manage triage calls/messages, support quality outcomes, and work systematically to ensure safe and effective care. Qualifications include a licensed Practical Nurse (LPN) with a diploma and active license, or an RN with an associate degree or higher, along with 2-3 years of preferred experience. Key skills encompass proficiency with Epic EMR, patient triage, computer competence, time management, and adaptability. Responsibilities involve partner collaboration, clinical triage, and contributing to quality care delivery. This role offers a full-time schedule with no nights, weekends, or holidays, and a hybrid work model after initial training.
