Primary Care Neighborhood Physician - Phoenix, AZ
Primary Responsibilities
- Deliver an enhanced level of care for patients with high hospitalization rates, frequent ER utilization, or complex medical and social needs.
- Provide dedicated care to Medicare Advantage patients for 60-90 days following hospital discharge, including addressing social determinants of health, referral and medication management, care gap assessments, and more.
- Manage primary care for complex patients while partnered with an interdisciplinary care team.
- Focus on patient needs with 60 minutes per appointment.
- Support patients and caregivers in confidently navigating the healthcare system and available resources.
- Improve clinical outcomes, patient experience, and quality of life through collaborative, team-based care.
Requirements
- Board Certified or Board Eligible in Internal Medicine, Geriatric Medicine or Family Medicine.
- Active and unrestricted Arizona medical license or ability to obtain prior to start date.
- Current and unrestricted DEA certificate or ability to obtain prior to start date.
- 3+ years of primary care physician experience.

Pay
The salary range for this role is $226,000 to $366,000 per year. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.
