Prevention Care Coordinator (VPV)

This position is dedicated to the Violence Prevention Villages (VPV) project, which aims to provide positive, proactive, and preventive upstream support and services to youth Pre-K–8 and their households. BUILD partners with schools to improve behavioral, social/emotional, and school outcomes for students, and engages the nonprofit ecosystem to support household wellbeing and stability. The role supports youth and households to improve long-term wellbeing, educational attainment, and economic stability across generations.

About the role

The Care Coordinator will conduct assessments, track progress, and identify and secure appropriate resources to support youth aged 3–15 and all members of their households. They will work with a team of direct service providers and leverage partner agencies to address the needs of selected youth and households, facilitating care coordination meetings for some cases. The role also involves developing additional partner agency relationships and utilizing assessment tools for initial and ongoing progress monitoring.

Work primarily takes place at assigned neighborhood school(s), the BUILD campus, in homes, and other locations as needed. The Care Coordinator reports directly to the Manager of Violence Prevention Villages.

Responsibilities

• Conduct initial assessments of youth, their household, and all members of their households to identify service opportunities and allocate services consistent with the VPV program model.
• Perform ongoing progress monitoring assessments to adjust service allocations, inputs, and providers as needed to sustain or increase progress, or taper off services if goals are achieved.
• Ensure appropriate type and levels of service provision.
• Collaborate with the VPV Manager to conduct CARE team/Case review team meetings, engaging BUILD staff, relevant school staff, and others to conduct shared assessment and planning for maximum benefit to the youth and household.
• Identify gaps in service array and work with the VPV Manager and/or other BUILD staff to build new partners to address those gaps.
• Contribute to client-related data systems for record keeping and reporting purposes.
• Contribute to reports as required per grant.
• Ensure the timely flow of case management activities to meet program service plan goals.
• Utilize the BUILD Universal Referral form to document and connect families to other BUILD services as needed.
• Operate professionally in the field on a flexible schedule, which may include evenings and weekends.
• Complete all assessments, service plans, case notes, and other significant program data in a timely manner.
• Maintain all required forms, including case notes, client service plans, referral forms, information releases, and any other necessary information in program participants' files while maintaining confidentiality.
• Establish a regular schedule of classroom and/or home visits, contacts, and follow-ups to build relationships with participants and ensure completion of service plans.
• Maintain active, open, and consistent communication with school counselors and other appropriate school staff to align services for youth and their household members.
• Represent BUILD to broaden community awareness and support, interpreting the agency’s programs and services at program-related community, coalition, and committee meetings as needed.
• Attend departmental and agency-wide staff meetings, as scheduled, and agency-wide events, as required.
• Maintain current knowledge in the field through participation in coursework, seminars, and/or workshops.
• Participate in agency-wide fundraising events.
• Participate in agency outreach and recruitment events, including evenings and weekends.
• Participate in civic engagement activities that advance the mission of BUILD, Inc.
• Perform other duties as assigned.

Requirements

• Minimum two years of experience working with inner-city families and household members in a human service setting, trauma-informed care, social service setting, or related field.
• Strong organizational skills.
• Excellent interpersonal, verbal, and written communication skills.
• An automobile is required along with current insurance and a valid driver’s license.
• Maintain respectful and appropriate relationships with program participants, all levels of agency staff, and the various publics of the agency, demonstrating sensitivity to all ethnic, racial, and economic backgrounds.
• Willingness to travel as needed for household visits.
• Passion for youth development work.
• Must successfully pass an extensive background check.
• Must be able to provide proof of vaccination against COVID-19.
