Prevention and Outreach Specialist
About the role
The Prevention & Outreach Specialist will conduct HIV testing, pre/post counseling, community outreach, referrals to social services, and facilitate groups. They will work with populations at higher risk for HIV infection; build partnerships with community partners; assist individuals to identify their risk for HIV infection or transmission, and how to change risky behaviors, and sustain positive risk reduction behavior changes; and conduct one-on-one and group education services.
Responsibilities
- Deliver all confirmatory test results and make appropriate referrals for follow up medical care and other services as identified.
- Contact appropriate referrals in a timely manner regarding services according to grant specifications.
- Maintain appropriate and required tracking of referrals (including PreP/nPeP, DOH, etc).
- Screen and identify barriers to care, refer to other services (i.e, mental health, housing), link clients to care providers, and monitor appointments and health outcomes.
- Develop a sustainable care plan with each client to provide and conduct appropriate interventions to all eligible individuals for DOH funded interventions.
- Complete appropriate and timely documentation for 1628s, plus all required documentation for DOH approved interventions.
Requirements
- Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
- One to Two years’ experience in HIV Services or HIV/STI Prevention Services or related health and human services field.
- Certifications (i.e. CPR): 500/501 HIV Testing and Training, Rapid Testing Certification, or the ability to obtain within six months of hire.
- Working knowledge of Microsoft Word, Excel, and basic computer software.
Qualifications
- Must have reliable transportation and insurance with valid license in good standing.
- Must be able to drive a 31’ mobile testing unit and have appropriate licensure and insurability.
