Join Samaritas as a Preservation Case Manager in the MiFamily Together (MiFT) program—a flexible, home-based model focused on family preservation and reunification.In this impactful role, you’ll work directly with families in their homes—helping reduce the risk of child maltreatment while promoting stability, wellbeing, and long-term success. Our work creates meaningful, lasting transformation.Join us and be the rock. #BeTheRockWhat You’ll DoAs a MiFT Case Manager, you’ll deliver hands-on, individualized support to families:In-Home Family Support Provide weekly, in-home services to families over a 6-month period Deliver crisis intervention, education, and support to families at risk of child removal or reunification Utilize Protective Factors and the 2Gen approach to strengthen family systemsAssessment & Treatment Planning Conduct comprehensive family assessments Collaboratively develop and implement goals with families Support skill-building and connect families to resourcesCollaboration & Advocacy Work closely with referral sources, courts, schools, and community providers Advocate for families and help coordinate needed services Actively participate in team meetings and case reviewsDocumentation & Compliance Maintain accurate, timely case records and documentation Track progress, complete reports, and meet program requirements Participate in ongoing training and apply best practicesWhat You Bring Bachelor’s degree in Social Work, Human Services, Education, or a related field required Experience working with children and families in a field-based or community setting preferred Knowledge of strength-based, family-centered, and solution-focused approaches preferred Ability to build trust, engage families, and navigate complex situationsWhat Sets You Apart Strong crisis intervention and problem-solving skills Ability to work independently while staying connected to a team Flexibility to meet families where they are—both literally and figuratively Passion for family preservation and long-term impactAdditional Requirements Valid driver’s license with a good driving record Reliable personal transportation suitable for transporting clients Current vehicle registration and proof of insurance Availability for evenings, weekends, and participation in a 24-hour on-call support rotationWhat to Expect Intensive, relationship-based work with families in their homes Flexible and varied schedule to meet family needs Opportunity to make a deep, lasting impact through extended engagementMake a Meaningful DifferenceIn this role, you won’t just support families—you’ll help transform futures.
