Job Summary
Provide a range of community and field-based prenatal outreach and care coordination to designated community sites in the Vista, Oceanside and San Marcos areas including outreach, health insurance education, and linkage to prenatal care. Assist in prenatal patient recruitment, program activities, and administrative support.
Essential Job Functions
- Create partnerships with local high schools, rehab facilities and other community programs for recruitment of prenatal patients
- Conduct community outreach, including giving presentations, and staffing at health fairs and events
- Compile statistics and reports on the number of outreach events, clients educated, clients referred to prenatal program and healthcare utilization of prenatal care with VCC
- Organize and present prenatal education for community presentations
- Attend community meetings, events, health fairs and other outreach opportunities
- Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
- Support the vision, mission and goals, and demonstrate commitment to the values of the organization
Minimum Qualifications
- Bachelor's degree in related field
- Minimum two years’ outreach and community education experience or equivalent combination of education and experience
- Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record
- Availability and willingness to work evening and weekend hours, as required
Preferred Qualifications
- Minimum four years‘ outreach and community education experience or equivalent combination of education and experience
Required Skills/Knowledge/Abilities
- Comfortable with initiating conversation with various populations regarding prenatal program & services
- Excellent verbal and written skills necessary for communication with patients/clients, providers and other staff
- Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
- Ability to prioritize and meet deadlines
- Ability and willingness to meet the organization’s attendance and dress code policies
- Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and Excel; Zoom
Salary Range
$23.00 - $25.00 per hour
