About the role

Provides telephonic triage and document review services requiring clinical judgement and interpretation, acting as a liaison between patients, care teams, and primary care providers.

Responsibilities

  • Reviews and triages telephone encounters, calls, and messages from patients and de-escalates or escalates to the appropriate care team or clinician
  • Prioritizes urgent medical triage to patients
  • Timely assessment for smart scheduling into clinician schedule
  • Reviews lab results, imaging results, consultation notes, and clinical summaries and identifies those needing to be directed to respective clinicians for review
  • Effectively communicates with appropriate disciplines involved in patient care, providing information and recommendations to providers
  • Develops professional working relationships with healthcare providers and facilities, fostering an open flow of information and support of home care goals
  • Promotes effective working relationships with nursing personnel, the management team, and healthcare providers
  • Determines individual patient needs for services, referrals, and training, and provides patient education as necessary on the plan of care
  • Attends required meetings and huddles
  • Sets up guidance and resources, when necessary, in care and treatment of patients
  • Provides guidance and direction to colleagues and acts as a resource/mentor for nonclinical staff
  • Captures team-based care in the absence of the Program Manager
  • Complies with federal and local confidentiality laws, including HIPAA, ensuring patient privacy
  • Adheres to Brown Health Medical Group Primary Care guidelines and policies for protecting patients' demographic, clinical, and financial information

Requirements

  • Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing
  • Education and Experience: Graduate of an accredited school of nursing, current Rhode Island RN license in good standing, 3-5 years of appropriate nursing experience

  • Proficiency in using technology with proficiency in electronic medical record (EMR) platforms and Microsoft Office applications such as: Outlook, Excel, and Word

  • Clear assessment, creativity, judgment, and decision-making skills
  • Initiative, organization, prioritization, and follow-through with work assignments to assure fulfillment of plans and goals

  • Maintaining quality, safety, and/or infection control standards
  • Performing standard clinical procedures according to established protocols
  • Maintaining professionalism and providing superior customer service to all

Physical Requirements

  • Works primarily in a remote capacity and has daily communications through virtual meetings, some time in the office may be required for meetings, trainings, etc.

Pay Range

$74,256.00-$148,512.00
