Responsibilities

• Develop and implement evidence-based care plans
• Conduct thorough assessments and develop treatment plans for patients
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Provide education and support to patients and families
• Participate in quality improvement initiatives

Requirements

• Bachelor’s degree in healthcare field required
• Minimum 2 years of relevant clinical experience
• Licenses/certifications as applicable (e.g., RN, LPN, etc.)
• Strong communication and interpersonal skills
• Proficiency in Microsoft Office Suite

Qualifications

• Experience with electronic medical records systems
• Knowledge of current best practices in patient care
• Ability to work independently and as part of a team
