About the Role

The Practical Nurse Supervisor or Resident Care Director (RCD) serves as the nursing leader, responsible for managing the resident care team and coordinating care and service delivery to residents in the community. The RCD acts as a liaison with hospital personnel, physicians, community organizations, and other health-related service agencies to provide optimal care. This role is accountable for oversight and implementation of resident care programs, performance management, and regulatory compliance in accordance with Federal, State, and Board of Nursing licensing requirements.

Requirements

  • Minimum of two (2) years of college in human services or health care field; holds an unencumbered state license as a professional Registered Nurse or Licensed Practical Nurse.
  • Minimum of two (2) years of experience providing resident care to the elderly or related experience with specialized memory care programs.
  • Minimum of one (1) year of nursing management experience, including hiring, coaching, performance management, scheduling, and daily resident care operations.
  • Experience with resident assessment and care planning.
  • Certified in CPR and first aid.
  • Must be willing to become trained and/or certified in state assessments and service plans as per regulatory licensing requirements.
  • Knowledge of infection control practices and prevention of disease transmission.
  • Ability to understand and conceptualize complex concepts and make appropriate decisions in stressful situations.
  • High-level communication skills, both verbal and written.
  • Ability to show compassion toward cognitively impaired older adults and demonstrate genuine concern for the physical and emotional needs of the elderly and their families.
  • Ability to work weekends, evenings, and flexible hours as needed for resident care and services, including on-call responsibilities.
  • Proficient in basic office equipment, including personal computers.
  • Valid and acceptable driving record.
  • Must be mobile and able to perform physical requirements of the job, including lifting greater than 50 pounds, extended periods of standing, walking, bending, lifting, pulling, and stooping.
  • This position is considered medium risk for exposure to COVID-19 due to direct care of potential COVID-positive residents.

Responsibilities

  • Model a business philosophy that treats every resident and family member as a personal customer.
  • Promote the health and wellness of residents by partnering with physicians, pharmacists, and other care providers to ensure an integrated and collaborative approach to care and service coordination.
  • Ensure provision of services to residents and validate compliance with all Federal and State policies governing resident care.
  • Collaborate with administration on referral development and achieving occupancy goals.
  • Establish rapport with residents, families, direct care associates, department managers, providers, vendors, and state/regulatory agency personnel.
  • Conduct routine resident care rounds to ensure needs are met in accordance with individualized care plans.
  • Follow up on information from shift reports, residents, or families regarding concerns.
  • Ensure completion of resident assessments at least twice per year or with changes in resident condition, as required by policy and regulations.
  • Responsible for appropriate retention of residents through tracking, trending, and reporting resident care quality data to identify risk.
  • Perform calculations for personnel actions, budgeting, and other financial responsibilities.
  • Recommend modifications to resident care staffing plans to ensure reasonable hours and acceptable working conditions while adhering to the established budget.
  • Deliver direct resident care and supervise direct care associates to ensure their safety and comfort.
  • Delegate assignments based on demonstrated skill capability and compliance with regulations.
  • Coordinate departmental schedules to ensure adequate staffing in accordance with company standards, policies, budgets, and state law.
  • Interview, hire, evaluate, motivate, and monitor the performance of care associates, focusing on candidates who model the company’s core values.
  • Conduct orientation for new resident care team members to ensure knowledge of expectations, policies, and procedures, as well as competency in resident care skills.
  • Oversee continuing education programs for direct care associates per company policy and regulatory requirements.
  • Hold resident care team associates accountable through coaching or corrective actions when necessary.
  • Provide education and training on resident care policies, procedures, and regulations as required.
  • Oversee the community medication management program to promote resident safety, including onboarding, training, observations, and performance reviews.
  • Conduct interviews and observations of residents to alert the Executive Director, family, and others of any changes in physical or emotional health.
  • Consult with other departments to provide an interdisciplinary approach to residents’ needs.
  • Measure outcomes, set goals, and lead the care team to achieve them.
  • Attend scheduled meetings, conferences, and training sessions as required for Commonwealth Senior Living and/or professional licensure.
  • Maintain regular communication with the Executive Director.
  • Strive to maintain a safe working environment through accident prevention, equipment preservation, and safe working practices.
  • Drive a company vehicle as required.
  • Perform other duties as assigned.
