About the role

The Carolina Assessment of Medications Program (CAMP) is the population health pharmacy team within the UNC Health System. The quality measure pharmacist will be responsible for UNC Health’s performance on value care contract measures related to quality measures prioritized by the UNC Health Alliance (e.g., statin use measures, polypharmacy-related measures, and medication adherence measures). The quality measure pharmacist will conduct in-depth chart reviews to assess the patient’s medication use experience, and engage in outreach to pharmacies, providers, and patients.
