Location: Remote
Status: PRN, hourly
Shift Hours: 8:00AM - 4:30PM
Weekend Work: None
On Call: No
Responsibilities
- Support the mission of the hospital and pharmacy department
- Notify the pharmacy/provider of missing prescriptions
- Request medication refills
- Affords assistance to the PHSO with medication-related quality measures for the PHSO
- Contact pharmacies to confirm prescriptions were received and picked up
- Call patients to remind them to pick up their medications
- Obtains medication lists and engages in discussions with patients about medication adherence to identify and resolve patient adherence barriers that do not require clinical intervention
- Request records from clinics and facilities as needed
- Will involve data analysis, e.g., payer data, claims, etc., research, and problem-solving to obtain resolution
- Sends referrals to other disciplines to coordinate care
Required Qualifications
- High School Graduate or Equivalent GED
- Required FL Registered Pharmacy Technician License
Preferred Qualifications
- 1 year Pharmacy Technician experience
- Patient Technician Certification (CPhT)
- PTCB certification required within 6 months of hire.
