Population Health Coordinator

Responsibilities

• Identifies patients through health plan gaps in care lists, state immunization databases, patient medical records, and patient disease management registries.
• Collaborates with providers to review and validate care needs against the medical record.
• Contacts patients with identified gaps in care to facilitate meeting these needs.
• Supports quality outcomes and high reliability in the Patient Centered Medical Home.

Organizational Responsibilities

• Attends and participates in department meetings and is accountable for all information shared.
• Completes mandatory education, annual competencies, and department-specific education within established timeframes.
• Maintains licenses/certifications in good standing throughout the fiscal year.
• Ensures adherence to regulatory agency requirements, survey processes, and compliance.
• Complies with established organization and department policies.
• Available to work overtime and additional shifts as required.

Education

• A high school diploma equivalency or one year of applicable cumulative job-specific experience is required.
• Required professional licensure/certification can be used in lieu of education or experience, if applicable.
