About the role
You will engage patients through motivational interviewing, use data to support care for high-risk populations, prepare clinical data for review, prepare workflows to support team-based care, and evaluate progress toward goals.
Responsibilities
- Coordinating high-risk patient management and outcomes
- Preparing and monitoring population health data
- Cooking communication and care across teams
Requirements
- Bachelor’s degree or equivalent professional experience
- 1-5 years’ experience preferred
- Experience in community health, patient resource coordination or equivalent
- Computer skills, clerical skills, data entry and organization
- Excellent written and verbal communication, and problem solving skills
- Able to read, write and speak the English language. A second language of Spanish is preferred
