About the role

You will engage patients through motivational interviewing, use data to support care for high-risk populations, prepare clinical data for review, prepare workflows to support team-based care, and evaluate progress toward goals.

Responsibilities

  • Coordinating high-risk patient management and outcomes
  • Preparing and monitoring population health data
  • Cooking communication and care across teams

Requirements

  • Bachelor’s degree or equivalent professional experience
  • 1-5 years’ experience preferred
  • Experience in community health, patient resource coordination or equivalent
  • Computer skills, clerical skills, data entry and organization
  • Excellent written and verbal communication, and problem solving skills
  • Able to read, write and speak the English language. A second language of Spanish is preferred
