Physician - Service Chief/Geriatrics and Extended Care
About the role
The New York Harbor VAMC, St. Albans Campus, is recruiting for a full time Service Chief for Geriatrics and Extended Care (CLC).
This position is accountable to the Executive Chief of Staff for the organization and delivery of intermediate inpatient and outpatient medical care provided at the ECC and its outpatient clinics.
This position may be eligible for a sign-on bonus for highly qualified candidates.
Responsibilities
Affirms alignment of extended care services with the facility's mission, vision, and strategic plan.
Ensures integration of medical care with other services and programs within and outside the CLC.
Directs and supervises the activities of the CLC, including both inpatient and outpatient services.
Ensures all CLC policies are effectively implemented and maintained and are updated as necessary.
Attends and provides input into, as appropriate, various committees and leadership teams.
Develops and implements a performance improvement plan.
Represents the Executive Chief of Staff and the Chief, Medical Service, as the physician at the CLC responsible for clinical administration and quality of patient care.
Develops policies, SOPs, and guidelines in cooperation with the Chief, Medical Service, and inpatient Geriatric Consultation Service at the Brooklyn Campus.
Ensures customer service standards are promoted throughout the St. Albans campus to include complaint resolution and service recovery.
Ensures comprehensive, coordinated, and timely long-term health care of excellent quality is provided for every geriatric Veteran and the St. Albans campus.
Provides primary care to an assigned panel of patients at the St. Albans outpatient clinic.
Assigns physician coverage for the nursing home units, designating alternates as necessary; ensures policies and procedures are in place to promote appropriate intermediate bed utilization.
Promotes effective communication and working relationships between the units of the St. Albans campus and the VA NYHHS's medical school and allied health affiliates.
Apprises patients and their families and/or significant others of significant findings, diagnoses and prognoses and collaborates with them on treatment and continuity of care plans and end-of-life planning; participates in interdisciplinary patient education delivery and documentation.
Serves as Director of the Interdisciplinary Long Term Care Conference.
Ensures competency assessment, service level orientation, in-service training and continuing education are provided for all St. Albans Provider staff on an ongoing basis, and that documentation of participation and evaluation is maintained.
Ensures that a clinical environment is maintained suitable for high quality geriatric education and training for students in the various health professions.
Responsible for resident education at the St. Albans campus.
Ensures appropriate supervision of house staff and students.
Evaluates teaching programs to determine effectiveness in meeting program goals and patient care needs.
Encourages research activities that add to a general understanding of the distinctive aspects of the diagnosis and treatment of disease and functional capacity.
Qualifications
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Medical Degree: Must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
License: Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
Board Certification: Board Certification/ Eligible in the specialty/sub-specialty of Internal Medicine is preferred.
Experience: Experience, training, or proficiency in Geriatrics, Long Term care, or Palliative Care is preferred.
