About the role

The Physician provides direct patient care in accordance with evidence-based treatment guidelines, delivering rheumatology care in a team-centered approach. This role involves interacting with patients and families to keep them informed and engaged in healthcare decisions, providing counseling and education on wellness and prevention, and referring patients to specialty care services as needed.

Responsibilities

  • Provide direct patient care at the practice site following evidence-based treatment guidelines in a rheumatology office setting.
  • Deliver specialized care and consultative services as requested by primary care and other subspecialist physicians.
  • Develop and work within a team approach for managing patients with chronic connective tissue and related disease states.
  • Interact with patients and families to keep them informed and engaged in healthcare decisions.
  • Provide counseling, education, and resource information on wellness, prevention of illness, and health maintenance.
  • Refer patients as appropriate for specialty care services and educational training for their conditions.
  • Use disease registries to identify gaps and out-of-range values in care delivery, utilizing support services and care managers to improve healthcare metrics.
  • Participate in improving access to care through process improvement, including same-day appointments and extended hours.
  • Work with the process team to improve efficiencies in care delivery, utilizing "top of license" care by the office team.
  • Maintain, monitor, and ensure accurate and timely documentation of patient encounter records to meet HH coding guidelines, CMS, HIPAA, and other governmental regulations.
  • Collaborate with district physician referral committees to provide appropriate and timely access to rheumatology care.
  • Participate in clinical integration within HonorHealth, including process improvement, working with care managers, and coordinating transitions of care to specialists and inpatient services.
  • Provide timely follow-up after inpatient discharge to ensure smooth transition of care to the outpatient setting.
  • Act in a fiscally responsible manner, delivering value (quality/cost) to patients by providing the right care by the right individual at the right time.
  • Regularly attend Physician Network meetings and events.

Requirements

  • M.D. or D.O. degree (Required).
  • Board Certified or Board Eligible in Rheumatology (Required).
  • Current Arizona or other compact state medical license (Required).
  • Basic Life Support (BLS) certification (Required).
  • Member of HonorHealth Medical Staff.
