Physician - Primary Care for SNFs
About the Role
Community Health Centers of America is hiring for an MD/DO to work in contracted skilled nursing facilities in the greater Yuba City, CA area. Must be willing to supervise NP/PAs. You will be responsible for diagnosing and treating patients in the SNF setting. This role is suited for Internal Medicine or Family Practice providers.

Responsibilities
- Evaluate medical conditions and help patients through diagnosis and treatment.
- Develop and implement treatment plans, including prescriptions, and monitor results.
- Interview patients, families, and others; conduct physical examinations; review medical histories; order and evaluate laboratory tests.
- Create and maintain interdisciplinary treatment plans by collaborating with primary care physicians, mental health therapists, nurses, and other providers.
- Ensure quality and safe service by enforcing policies, procedures, standards, and legal regulations, including Joint Commission requirements.
- Maintain accurate historical records by documenting symptoms, medications, treatment events, and writing summaries.
- Stay current with medical knowledge through continuing education, professional publications, and networking.
- Enhance department and organization reputation by accepting new challenges and adding value to the role.
Requirements
- Degree in medicine and a valid license from the California Medical Board.
- 3-5 years of related experience preferred; or equivalent combination of education and experience.
- Current BLS/ACLS and DEA certifications required.
- In-depth knowledge of legal medical guidelines and best practices.

Pay
$300,000.00 - $350,000.00 per year, depending on experience.
Schedule
- Day shift.
- Monday to Friday.
- No nights or weekends.
- Night/weekend call coverage provided.

In person at contracted skilled nursing facilities in the greater Yuba City, CA area.
