Physician - Primary Care for SNFs

Community Health Centers of America is seeking an MD/DO to provide care in contracted skilled nursing facilities (SNFs) in the greater Sacramento, CA area. This role involves diagnosing and treating patients in the SNF setting and supervising Nurse Practitioners (NPs) and Physician Assistants (PAs).

Responsibilities

  • Evaluate medical conditions and develop and implement treatment plans, including prescriptions.
  • Monitor and evaluate treatment results.
  • Interview patients, families, and other relevant parties; conduct physical examinations; observe behaviors; review medical history; order and evaluate laboratory tests.
  • Determine the nature and extent of medical conditions to create treatment plans.
  • Collaborate with primary care physicians, mental health therapists, nurses, and other healthcare providers to ensure interdisciplinary treatment.
  • Enforce policies, procedures, standards, and legal regulations to ensure quality and safe service for patients and staff.
  • Participate in utilization reviews and remain available for emergency consultations.
  • Document symptoms, medications, treatment events, and write summaries to maintain historical records.
  • Stay updated with continuing medical education, professional publications, and networking opportunities.
  • Accept ownership for new requests and explore opportunities to add value to job accomplishments.

Requirements

  • Completion of residency.
  • Valid California state medical license as MD or DO.
  • Valid DEA certificate.
  • 3 years’ experience working as a Physician.
  • Current BLS and/or ACLS certification.
  • In-depth knowledge of legal medical guidelines and best practices.

Qualifications

  • Degree in medicine and valid license from the California Medical Board.
  • 3-5 years of related experience preferred; equivalent combination of education and experience considered.

Pay

$300,000.00 - $350,000.00 per year, depending on experience.

Schedule

  • Day shift.
  • Monday to Friday.
  • No nights or weekends.
  • Night/weekend call coverage provided.
