About the role
The positions are located in the Primary Care Service at the Eastern Colorado Health Care System and provide support to the primary care of Veterans and their significant others within the ECHCS catchment area. Our Primary Care Provider functions as a team provider in a shared workflow with a multidisciplinary Patient-Aligned Care Team (PACT - a medical home model).
Responsibilities
- Functions as a team provider in a shared workflow with a multidisciplinary Patient-Aligned Care Team (PACT - a medical home model)
- Performs histories and physicals
- Initiates and adjusts therapeutic regimens
- Orders medications/supplies in accordance with VA policy and the VA formulary
- Provides appropriate follow up care
- Triage patients with non-medicine complaints, diseases, and injuries to appropriate disciplines for diagnosis and treatment
- Comfort with chronic disease management and geriatric patients
- Comfort in providing pain management
- Comfort in delivering mental health treatment to our Veteran population
- Comfort in providing women's health care
- Communicates with and utilizes other disciplines (e.g. - nursing - surgery) to facilitate treatment planning and implementation
- Collaborates with divergent multidisciplinary team members in a manner that enhances coordination of comprehensive patient care
- Participates in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients
- Provides health education and health coaching on wellness, disease prevention, chronic care management, and self-management skills to patients and personal support persons
Requirements
- Full and unrestricted license to practice medicine or surgery in a State - Territory - or Commonwealth of the United States - or in the District of Columbia
- Completed residency training approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification
- Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation

- Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met.
