Qualifications

To qualify for this position - you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement.

Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.

Physical Requirements

This position requires visual acuity - keen hearing - clear distinctive speech - and manual dexterity.

This position requires potentially long periods of continued walking - standing - stooping - sitting - bending - pulling - and pushing.

Transferring patients and objects may be required.

The incumbent may be exposed to infected patients and contaminated materials and may be required to wear protective clothing in isolation situations or operative/invasive procedures.

The incumbent may occasionally be exposed to patients who are combative secondary to delirium - dementia - or psychiatric disorders.

The incumbent must be a mature - flexible - sensible individual capable of working effectively in stressful situations - able to shift priorities based on patient needs.

Must be able to perform CPR per ACLS/BLS protocols.

Duties

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 25th - 2026.

The initial cut-off date for referral of eligible applications will be Monday - May 18th - 2026 with subsequent cut-off dates every 2 weeks.

Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available.

• Provide direct patient care to include - but are not limited to history and physicals - medication management - evaluation of diagnostic tests and treatment planning - clinical reminder completion as applicable.
• Function as a leader in the PACT approach in the delivery of care - which emphasizes a team-based approach to healthcare.
• Oversee PACTs compliance to MyHealtheVet.
• Ongoing - continuous care of one or more assigned panel(s) of Veteran patients.
• Utilizing all available tools - such as registries - to enable effective and efficient identification and intervention of individual patients and cohorts.
• Ensuring appropriate evaluation and access is provided to patients assigned to the patient panel.
• Functioning at the full extent of the team member's relevant clinical privileges - credentials - scopes of practice - elements of practice - certification - functional statement - or other VHA or local facility approved documentation of competency.
• Participating in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients.
• Implementing primary care operations management processes - as appropriate.
• Providing health education and health coaching on wellness - disease prevention - chronic care management - and self-management skills to patients and personal support persons - commensurate with the documented expertise or professional training of the PACT member.
• Engaging patients in using health care - encouraging patients to engage personal support persons - receiving input from patients and personal support persons regarding VA care.
• Using formal and informal communications that are respectful - effective - timely - and bidirectional with all team members to convey significant - clinically relevant information for the care of the patient.
