About the Role

The Practice Manager provides operational leadership, performance management, and infrastructure oversight for the long-term care (LTC) physician and advanced practice provider (APP) group across Georgia. This role ensures consistent clinical coverage, provider productivity, regulatory compliance, and alignment with facility operations and value-based care models, including Medicare Advantage and I SNP populations. The Manager partners closely with Medical Directors, APPs, facility leadership, and corporate stakeholders to drive quality outcomes, operational efficiency, and growth in the Georgia market.

Responsibilities

  • Provider Operations & Coverage
    • Oversee daily operations of physician and NP teams across Georgia facilities.
    • Ensure consistent weekday clinical coverage.
    • Manage scheduling and territory alignment, coverage gaps, contingency plans, call-out response, and escalation.
  • Productivity & Performance Management
    • Monitor and manage provider productivity benchmarks (encounters, efficiency), timeliness of rounding and documentation, and attendance and reliability.
    • Implement performance improvement plans, coaching, and corrective actions when necessary.
  • Facility & Stakeholder Alignment
    • Serve as liaison between Providers, Facility Administrators, Directors of Nursing (DONs), and Corporate leadership.
    • Resolve workflow inefficiencies, communication breakdowns, and operational escalation issues.
  • Workforce Management
    • Lead recruitment, onboarding, and retention strategies.
    • Manage PTO, scheduling transitions, and provider performance issues.
    • Ensure collaborative agreements comply with Georgia regulations, and credentialing and onboarding processes are completed.
  • Clinical Operations Support (Non-Clinical Leadership)
    • Partner with medical leadership to standardize documentation expectations and support appropriate coding (HCC/RAF).
    • Identify variation in care processes involving falls management, rehospitalization trends, and psychotropic medication management.
  • Quality & Utilization Performance
    • Drive performance on rehospitalization reduction, preventive care gap closure, and length of stay and utilization trends.
    • Analyze and act on facility-level data and provider-level performance.
  • Regulatory & Compliance Oversight
    • Ensure adherence to CMS LTC requirements, Medicare Part B billing standards, and Georgia scope-of-practice regulations.
    • Support Survey readiness and documentation defensibility.
  • Financial & Growth Alignment
    • Partner with Physician Services leadership to support financial performance, provider productivity, and sustainable growth.
    • Monitor provider productivity trends, encounter volume, documentation timeliness, and billing readiness to support accurate and timely revenue capture.
    • Identify opportunities to improve provider deployment, facility coverage, and market expansion based on census, payer mix, facility needs, and value-based care priorities.
    • Collaborate with billing, coding, credentialing, and operations teams to address workflow barriers impacting charge capture, documentation completion, or provider onboarding.
    • Support growth initiatives related to new facility coverage, provider staffing models, and alignment with value-based care strategies.

Qualifications

  • Education
    • Bachelor’s degree in healthcare administration, business administration, nursing, or a related field required.
    • Master’s degree in healthcare administration, business administration, nursing, or related field preferred.
  • Experience
    • Physician (MD/DO), Nurse Practitioner, or Healthcare Executive (MBA/MHA preferred).
    • 5+ years of experience in:
      • Long-term care or post-acute care.
      • Provider group operations.
      • Value-based care environments/models.
    • Strong knowledge of:
      • Medicare Part B Billing.
      • HCC/RAF coding.
      • SNF/LTC operations.
  • Licensure/Certification
    • No clinical license is required. If the selected candidate is a licensed clinician, they must maintain an active, unrestricted license in the applicable state of practice and remain in good standing with all applicable licensing boards and regulatory agencies.

  • Foundational understanding of medical practice operation and billing documentation.
  • Six Sigma certification or other operational leadership certifications.

Schedule

Full-time, 1st Shift.
