Position Duties
- Monitor, examine and provide follow-up treatment to patients, primarily the elderly, who are being discharged to skilled care or nursing facilities for short-term rehabilitation after treatment for acute conditions.
- Provide specialized palliative care and treatment to patients who require pain management or other palliative care as part of their follow-up care and treatment.
- Cook up follow-up care with physician specialists at medical centers who treated the acute condition.
Position Qualifications
- Doctor of Medicine or Doctor of Osteopathy degree required. Foreign equivalent degree accepted.
- Must have a total of 60 months of training as follows:
- (1) 36 months of training in a residency in either internal medicine or family medicine;
- (2) 12 months of training in a fellowship in geriatric medicine;
- (3) 12 months of training in a fellowship in palliative medicine.
- Must be board certified or eligible for board certification in all three of the following areas:
- (1) internal medicine or family medicine;
- (2) geriatric medicine;
- (3) palliative medicine.
- Must have valid State of Hawaii medical license.
- Will be required to examine and treat patients at nursing homes across the Honolulu Metropolitan Statistical Area (City & County of Honolulu).
