About the role

This field-based position serves as the primary connection between Giving Home Health Care, our patients, and their healthcare providers. The ideal candidate combines medical-office knowledge with strong relationship-building and sales skills. This individual must be comfortable meeting with physicians and clinical staff, attending patient appointments, communicating persuasively, managing a high volume of paperwork, and following requests through to completion. This position requires independence, consistent follow-up, local and regional travel, and a strong commitment to achieving measurable results.

Responsibilities

  • Patient Advocacy and Appointment Support
    • Attend medical appointments with patients to help communicate their needs and advocate for provider support
    • Coordinate and schedule medical appointments necessary to support patient care and benefit-related documentation
    • Communicate professionally and compassionately with patients throughout the documentation process
    • Help patients understand appointment expectations and any paperwork needed for their visit
    • Partner with patients, providers, and internal teams to address barriers that could delay care or required documentation
  • Provider Relationships and Business Development
    • Build and maintain strong relationships with physicians, advanced practice providers, clinic managers, and medical-office staff
    • Serve as the primary point of contact for assigned provider offices and accounts
    • Identify and cultivate new provider relationships that can support Giving Home Health Care patients
    • Educate provider offices on Giving Home Health Care’s services, patient population, and documentation processes
    • Present requests clearly, confidently, and persuasively while respecting each provider’s independent medical judgment
    • Conduct regular in-person visits, follow-ups, and relationship-building activities with assigned provider offices
    • Maintain a consistent pipeline of provider appointments and relationship-development opportunities
    • Represent Giving Home Health Care professionally within the local medical community
  • Medical Documentation and Follow-Up
    • Facilitate the review and completion of appropriate medical paperwork with providers and clinics
    • Coordinate documentation requests involving medical necessity, plan-of-care paperwork, medical equipment, claim-related letters, and other patient-supporting records
    • Ensure providers receive accurate, organized, and complete documentation before scheduled appointments
    • Follow up consistently with provider offices to obtain completed paperwork within required timelines
    • Monitor outstanding requests and prioritize items based on urgency, patient need, and applicable deadlines
    • Review returned paperwork for completeness before submitting it to the appropriate internal department
    • Maintain accurate notes, appointment details, provider contacts, and documentation statuses in company systems
    • Protect patient information and follow all HIPAA, privacy, and company compliance requirements
  • Internal Collaboration and Performance
    • Partner with clinical teams, technical writers, patient-care teams, operations staff, and leadership to move patient requests forward
    • Communicate delays, provider concerns, and potential barriers proactively
    • Manage multiple provider accounts, appointments, deadlines, and documentation requests simultaneously
    • Work effectively in a fast-paced, high-volume environment while maintaining accuracy and professionalism
    • Meet established performance expectations related to appointments, provider relationships, completed documentation, turnaround time, and other assigned goals
    • Take ownership of assigned accounts and follow each request through completion
    • Participate in team meetings, training, and market-development initiatives

  • Ability to maintain professionalism when navigating objections, delays, or difficult conversations
  • Willingness to collaborate and contribute to a team-oriented environment

Requirements

  • High school diploma or equivalent required; additional education in healthcare, business, marketing, or a related field is preferred
  • This is a field-based position requiring consistent travel within approximately one to two hours of the Albuquerque office
  • Employees must use their own reliable vehicle for work-related travel
  • A valid driver’s license, current automobile insurance, and an acceptable driving record are required
  • Mileage for approved work-related travel is reimbursed at $0.76 per mile
  • Travel outside the immediate Albuquerque area may be required based on patient and provider needs
