About the role

Taking part in community health is rewarding and meaningful. LifeLong Medical Care is looking for a Physician or Advanced Practitioner committed to providing high-quality care to patients seen in our Homeless Services Department. The ideal candidate must be dedicated to building a collaborative team model of care and have experience working with the under-served.

Responsibilities

  • Provides preventive, urgent care and primary care health services.
  • Performs clinical assessment of patients, including ordering appropriate diagnostic evaluations, and plans a program of care.
  • Provides verbal and written education and counseling to patients, families, and caregivers.
  • Maintains patient records necessary for clinic reporting, billing and quality improvement.
  • Functions potentially in a variety of settings including health center, skilled nursing facilities, supportive housing sites, and in clients' homes.
  • Participates as a member of the site clinical services team and attends clinical team meetings.
  • Provides specialty care services, clinical case management and team participation to designated programs (e.g., HIV services, women’s health, house calls, diabetes, Bridge, etc.).
  • Captors care with other health team members including case management and behavioral health staff.
  • Acts as an advocate for the patient and liaison with outside agencies as appropriate.
  • Physicians supervise Advanced Practice Providers.
  • Participates in after-hours on-call as needed.
  • Assists in the supervision and teaching of medical residents, students or clinical fellows.
  • Actively participates in internal quality improvement teams and works with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards.

Qualifications

  • MD/DO with $203,652.80 - $225,118.40 or NP/PA $127,504 - $151,153.60/year depending on years since licensed and pro-rated to FTE.
