Physician, Endocrinologist FT Licensed in CT, DE, ME, NH, NJ, PA, RI, or VT
About the Role
We are seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity, and to collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care. Work is remote, and schedules can be flexible. This is a Modified Full Time (40 hours per week) position based in a remote, telemedicine clinical setting for physicians licensed in CT, DE, ME, NH, NJ, PA, RI, or VT.
This role will include participation in a rotating after-hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage. The physician will work closely with a supportive care team and have protected time for administrative duties.
Responsibilities
- Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management.
- Manage your own patient panel and collaborate with Registered Dietitians. (Currently, this role does not collaborate with APPs, but in the future, the physician will conduct chart reviews and return visits for APP patients every third visit to ensure quality and continuity of care.)
- Provide treatment planning and medical management support for complex cases or escalated concerns.
- Prescribe non-controlled obesity medications for patients under direct care.
- Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
- Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education. This role may include APP collaboration and supervision responsibilities.
- Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
- Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.
Requirements
- MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate or board certified in Endocrinology.
- Licensed to practice and prescribe legally in CT, DE, ME, NH, NJ, PA, RI, or VT.
- Must hold an LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact).
- Are or have in the past been credentialed with insurance payers (strong preference for multiple state licenses).
- Excellent interpersonal skills for patient and team interactions.
- Committed to the highest quality of medical care and an evidence-based approach to obesity management.
- Prior experience conducting telemedicine visits and a great "web-side" manner.
- Prior experience working collaboratively with advanced practice professionals and registered dietitians.
Bilingual candidates with medical fluency in Spanish are highly preferred.

Pay
Base Annual Salary: $210,000—$230,000 USD
