About the role
Physician Assistant responsible for conducting in-home and virtual clinical evaluations for vulnerable patient populations.
Key Responsibilities
- Perform comprehensive patient histories and physical examinations.
- Manage chronic conditions and reconcile medications.
- Develop and implement evidence-based care plans aligned with value-based care objectives.
- Coordinate follow-up care in collaboration with interdisciplinary teams.
- Utilize data and analytics to inform clinical decisions.
- Document all patient interactions accurately in electronic health record (EHR) systems.
Qualifications & Skills
- Clinical experience in patient evaluation and management.
- Strong understanding of chronic disease management and medication reconciliation.
- Ability to develop evidence-based care plans.
- Excellent communication and teamwork skills.
