About the role
Continuum Rehab Group provides in-house therapy with Skilled Nursing facilities in Texas, New Mexico, Indiana, Arizona, and Missouri. Responsibilities
- Utilizes evidence-based practice to support clinical interventions.
- Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist.
- Provides skilled therapy services to patients who need rehabilitation services.
- Affirms and documents patient's progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
- Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
- Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
Qualifications
This position is open to therapists at all experience levels. Required qualifications include a current license as a Physical Therapy Assistant.
