Physical Therapy Assistant - Home Care

About the Role

Provides physical therapy interventions under supervision, assisting with patient care, monitoring progress, and documenting treatment per policy. Ensures adherence to scope of practice, modifies treatment plans as directed, and collaborates with supervising therapists.

Responsibilities

• Manages caseload and delivers therapy services at patients' homes or facilities
• Assists with activities of daily living and helps patients with therapeutic activities and equipment
• Participates in team meetings and special projects
• Travels using approved transportation and handles equipment safely

Requirements

• NY State Certification as a Physical Therapist Assistant
• Associate’s degree from an approved program
• Minimum two years of clinical experience
• Valid driver’s license may be required
• Ability to perform physical tasks, assist with mobility, and use medical equipment
• Compliance with policies, flexibility, and community engagement

Qualifications & Skills

• Bilingual skills may be preferred
• Basic PC skills are advantageous
